After you leave your employer

You will lose your eligibility for membership in the Health Insurance Society after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
  • Eligibility Certificate for Ceiling-Amount Application(if issued)
  • Certificates Issued for Specific Disease Treatment(if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents
Address inquiries to: The person in charge of social insurance at the establishment
Notes:
  • ※Insurance cards cannot be used starting the day after the date of retirement.

If you wish to remain a member of the Health Insurance Society

Required documents:
Deadline: Within 20 days after the date of loss of eligibility of the insured person
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment
Address inquiries to: The person in charge of social insurance at the establishment from which you retired
Submit to: The person in charge of social insurance at the establishment from which you retired
Notes: When you submit an application form, a new insurance card (with a changed insurance-card symbol / number) and insurance-premium notice of payment will be delivered to your home by the Yazaki Health Insurance Society. To complete the Voluntarily and Continuously Insured Person procedure and obtain eligibility, you must pay this premium by the 10th of the corresponding month.
